The Barbie BBL is ICON's signature torso-and-glute contouring procedure, performed awake in our Birmingham surgical suite by Dr. Zayid. It combines liposuction of the full abdomen, flanks, and lower back with fat transfer to the buttocks for projection and to the hip dips for fullness. Removing fat from the entire waistline and placing it exactly where the silhouette needs it snatches the waist, improves the waist-to-hip ratio, and produces a natural, desirable, feminine contour with an athletic look rather than an exaggerated one.
What the Procedure Includes
Full-torso liposuction. The full abdomen, both flanks, and the lower back are treated in one session. This 360-degree approach is what narrows the waist from every angle and creates definition through the abdomen; treating the front alone cannot produce the same ratio.
Fat transfer to the buttocks. The fat removed from your waistline is purified and transferred to the buttocks to add projection and shape. Because it is your own tissue, the result looks and moves naturally.
Hip dip augmentation. Fat is also placed into the hip dips, the indentations between the hip bone and the thigh, so the line from waist to hip to thigh reads as one smooth, continuous curve.
Ultrasound guidance. Every fat transfer at ICON is performed under ultrasound guidance. Seeing the plane of injection in real time lets Dr. Zayid place fat precisely where it belongs and keeps the procedure safe.
Awake, Safe, and In Office
The entire procedure is performed awake using customized tumescent local anesthesia, the same approach behind the more than 1,000 awake liposuction procedures Dr. Zayid has performed since 2023. Avoiding general anesthesia removes its risks and its recovery, and allows you to be positioned naturally during the procedure so contours are checked in real time.
Recovery and Compression
Expect about three days of downtime before returning to normal daily activity. A compression garment is mandatory for the two months after the procedure. It is not optional and it is not a formality: consistent compression is what allows the contour and the skin tightening to heal and settle correctly, and it protects the shape of the new waistline while the transferred fat establishes its blood supply. You will receive detailed garment and activity instructions, including how to sit and sleep in the early weeks, and you will be seen in the office for follow-up.
The Result
A beautiful, natural silhouette: a defined, snatched waist and abdomen, filled hip dips, and a rounder, more projected glute that together give an athletic, feminine build.
