1. Acceptance of Terms
By accessing or using the website and services of ICON Anti-Aging and Aesthetics PLLC (“ICON,” “we,” “us,” or “our”), you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our website or services.
2. Services
ICON provides aesthetic, anti-aging, and wellness treatments including but not limited to injectable treatments, laser procedures, body contouring, weight loss programs, hormone optimization, and skincare services. All treatments are performed by licensed medical professionals under the supervision of a board-certified physician.
3. Medical Disclaimer
The information provided on this website is for general informational purposes only and does not constitute medical advice. It is not intended to be a substitute for professional medical advice, diagnosis, or treatment. 4. Appointment, Deposit, Cancellation, and No-Show Policy
Deposits. A deposit is required to reserve an appointment at ICON Anti-Aging & Aesthetics. By providing a payment card to book an appointment, you authorize ICON to charge the deposit to that card at the time of booking and to apply the deposit to the cost of your treatment when you attend your appointment.
48-hour cancellation window. Appointments may be cancelled or rescheduled without charge if you notify us 48 hours or more before the scheduled appointment time. Any cancellation or reschedule made within 48 hours of the appointment time, and any appointment that is missed without notice (a "no-show"), is subject to a late cancellation / no-show fee equal to the deposit on file. By booking, you agree that the deposit will be retained as this fee, and you authorize ICON to charge the card on file for the fee if a deposit was not collected at booking.
Fees are non-refundable and not subject to dispute. Late cancellation and no-show fees are a charge for the reserved appointment time, which is held exclusively for you and cannot be offered to another patient on short notice. These fees are non-refundable. By booking an appointment, you acknowledge this policy, agree that the fee is a valid and authorized charge, and agree not to dispute or charge back the fee with your card issuer. Any chargeback or dispute of a fee charged in accordance with this policy will be contested by ICON using this agreement, the booking record, and the appointment confirmation and reminder communications sent to you.
Waivers and credits. ICON may, at its sole discretion, waive a late cancellation or no-show fee, or hold the amount on your account as a credit toward a future appointment. A waiver or credit in one instance does not change this policy or create an obligation to waive fees in the future.
Right to refuse service. ICON reserves the right to decline to schedule or treat any patient who has multiple no-shows, late cancellations, or payment disputes with the practice, and may require prepayment in full for any future appointments.
Consultation fees. Any consultation fee you pay is credited toward a treatment booked or products purchased on the same day.
Acknowledgment. This policy is presented at the time of booking, is included in your appointment confirmation and reminder messages, and is available at all times on this page. Booking an appointment by any method, including online, by phone, by text, or in person, constitutes your acceptance of this policy.
5. Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept major credit cards, debit cards, and other forms of payment as specified at the time of booking. Prices for treatments are subject to change without notice.
6. Refund Policy
Due to the nature of medical aesthetic treatments, refunds are generally not provided for completed treatments. If you have concerns about your treatment results, please contact our office to discuss your options. Unused treatment packages or prepaid services may be eligible for a partial refund at our discretion.
Deposits retained as late cancellation or no-show fees under Section 4 are non-refundable and are not eligible for dispute or chargeback.
7. Informed Consent
Before any treatment, you will be required to sign an informed consent form acknowledging the potential risks, benefits, and alternatives to the proposed treatment. You have the right to ask questions and refuse any treatment at any time.
8. Intellectual Property
All content on this website, including text, graphics, logos, images, and software, is the property of ICON Anti-Aging and Aesthetics PLLC and is protected by copyright and intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written permission.
9. Limitation of Liability
ICON Anti-Aging and Aesthetics PLLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of our website or services. Our total liability shall not exceed the amount paid for the specific treatment or service giving rise to the claim.
10. Privacy

11. SMS Terms & Conditions
These SMS Terms & Conditions govern text message (SMS) communications between you and ICON Anti-Aging and Aesthetics PLLC.
1. SMS Consent Communication. The information (phone numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
2. Types of SMS Communications. If you have consented to receive text messages from ICON, you may receive messages related to appointment reminders, follow-up messages, billing inquiries, and promotions or offers. Example: “Hello, this is a friendly reminder of your upcoming appointment with ICON Anti-Aging & Aesthetics on [Date] at [Time]. Reply STOP to opt out of SMS messaging at any time.”
3. Message Frequency. Message frequency may vary depending on the type of communication. For example, you may receive up to 2 SMS messages per week related to your appointments or account status.
4. Potential Fees for SMS Messaging. Standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.
5. Opt-In Method. You may opt in to receive SMS messages from ICON verbally during a conversation, by submitting an online form, or by filling out a paper form.
6. Opt-Out Method. You can opt out of receiving SMS messages at any time by replying “STOP” to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.
7. Help. If you are experiencing any issues, you can reply with the keyword “HELP,” or get help directly from us by calling (248) 436-4002. If you do not wish to receive SMS messages, you can choose not to check the SMS consent box on our forms.
