What does the physician review first?
Your intake: a detailed health history covering past and current conditions, every medication and supplement you take, prior weight loss attempts, and family history. Several items in that history are decisive on their own. A personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2, a history of pancreatitis, active gallbladder disease, pregnancy or plans to become pregnant, and certain eating disorders are reasons a physician will not prescribe GLP-1 medication, regardless of weight.
Why are labs required before starting?
Because the medication affects systems that need a baseline. Labs typically assess metabolic health, kidney and liver function, thyroid function, blood sugar markers, and lipids. They can reveal conditions that change the plan, such as a thyroid issue driving weight gain that deserves its own treatment, or kidney function that requires caution. They also establish the starting point against which progress and safety are measured. At ICON, labs come before any prescription, without exception.
What makes someone a good candidate?
Generally, a BMI in the range where medication is indicated, or a lower BMI with weight-related conditions such as prediabetes, high blood pressure, or elevated lipids. Beyond the numbers, good candidates have tried lifestyle changes without lasting success, understand that medication supports rather than replaces nutrition and strength training, and are willing to attend follow-up visits and repeat labs. Motivation to protect muscle mass during weight loss is a strong predictor of a good outcome.
When will a physician say no?
When a contraindication is present, when labs reveal a problem that needs to be addressed first, when the weight concern is better explained by another condition, or when the patient isn't in a position to commit to monitoring. A physician may also decline to prescribe to someone who is already at a healthy weight and seeking cosmetic weight loss. Being told no is not a rejection; it is the evaluation working as intended, and it usually comes with a recommendation for what would help instead.
What does ongoing supervision involve?
Doses are started low and increased gradually to minimize side effects. Follow-up visits track progress, side effects, and body composition, with the goal of losing fat rather than muscle. Repeat labs confirm the medication remains safe to continue. Refills are not automatic; each one follows a physician's review. That structure is the difference between medically supervised weight loss and simply obtaining a medication.
