SHBG, or sex hormone binding globulin, is a protein produced mainly by the liver that binds to testosterone and estrogen in the bloodstream, making that bound portion biologically unavailable for your cells to use. Because of this, two people with identical total testosterone or estrogen levels can have very different amounts of usable, "free" hormone, depending on their SHBG level — which is exactly why SHBG is such an important, and often overlooked, piece of a complete hormone evaluation.
Why "total" hormone levels don't tell the whole story?
When a hormone is bound to SHBG, it's effectively locked away, unable to bind to receptors and produce its usual effects in the body. Only the unbound, or "free," portion of a hormone is available to act on tissue. This means a total testosterone reading that looks perfectly normal on paper can still come with real, symptomatic low testosterone if SHBG is elevated and binding up a large share of it — and, less intuitively, a total level that looks technically low can sometimes come with adequate free hormone if SHBG is on the lower end.
What affects SHBG levels?
Several factors influence how much SHBG your liver produces. SHBG tends to rise with age, which is part of why free testosterone often declines even faster than total testosterone in aging men. Thyroid function, liver health, certain medications (including oral estrogen and some anti-seizure medications), and body composition all influence SHBG levels as well — higher body fat, for instance, is generally associated with lower SHBG, while conditions like hyperthyroidism tend to raise it.
Why this matters for treatment decisions?
A complete hormone evaluation should include SHBG alongside total and free (or calculated free) testosterone or estrogen, rather than relying on total levels alone. Without SHBG in the picture, it's possible to either miss a real, symptomatic hormone deficiency because the total number looks acceptable, or to over-treat a patient whose total number looks low but whose free hormone is actually adequate. This is part of why an experienced provider looks at your full lab panel and your symptoms together, rather than reacting to a single isolated number.
What symptoms might point to an SHBG issue?
If you have persistent symptoms of low testosterone or estrogen — low energy, reduced libido, mood changes — despite a total hormone level that looks technically normal, elevated SHBG is one of the more common, and more commonly missed, explanations. Conversely, unexplained symptoms alongside a "low" total reading are worth investigating with a free hormone measurement before assuming treatment is automatically warranted.
How do you get the full picture?
SHBG is one of those lab values that rarely gets ordered by default but often changes the entire interpretation of a hormone panel once it's included. If your prior labs only included total testosterone or total estrogen, it's worth asking your provider whether a more complete panel, including SHBG, would give a clearer picture of what's actually happening in your body.
