General Appointment Cancellation Policy
– Cancellations or Changes Within 48 Hours:
Any cancellation or appointment change that occurs within 48 hours of the scheduled appointment will incur a minimum fee of $100.
– For procedures requiring additional staffing, pre-procedure medications, or customized supplies, cancellation or rescheduling within 48 hours may result in higher fees proportional to the costs incurred.
– We reserve the right to determine the final fee based on the nature of the appointment and pre-scheduled resources.

Credit Card Requirement
– All appointments require a valid credit card on file in order to be booked.

Payment Method
– The applicable cancellation fee will be automatically charged to the card on file.
– Please ensure your payment information is up to date to avoid any inconvenience.

Weight Loss Memberships
– No Cancellation/Refund Policy:
Weight loss memberships are subject to a no-cancellation, no-refund policy under any circumstances, including but not limited to dissatisfaction with results or unforeseen personal circumstances.
– Disclaimer: Results vary by individual, and we do not guarantee specific outcomes. Lack of results does not qualify for refunds or adjustments to membership fees.
– Shipping Address Confirmation: After placing an order, members must text 248-436-4006 to confirm their shipping address. ICON Anti-Aging and Aesthetics PLLC is not responsible for any delivery issues, delays, or losses if the shipping address is not confirmed via text after ordering.

Hormone/Peptide Memberships
– No Guarantee of Results:
Hormone and peptide memberships do not guarantee specific results, as individual outcomes may vary.
– 90-Day Cancellation Notice:
These memberships require a 90-day lead time for cancellation. Failure to notify us in writing 90 days prior to cancellation will result in continued charges to the payment method on file.
– The payment method on file will be charged for any remaining fees up to 90 days before cancellation is fully processed.

ICON Memberships (Including Beauty Bank Memberships)
– 90-Day Cancellation Notice:
ICON memberships, including beauty bank memberships, require a 90-day notice prior to cancellation.
– Failure to notify us in writing 90 days in advance will result in continued charges to the payment method on file. Payments for up to 90 days will be collected to account for the notice period.

Gift Cards, Memberships, and Packages
– Non-Transferable and Non-Refundable:
All gift cards, memberships, and packages are non-transferable and cannot be redeemed for cash value under any circumstances.
– These services are for the exclusive use of the patient to whom they were issued.

Giveaway Prizes
– No Cash Value Redemption:
Giveaway prizes cannot be redeemed for cash value. Prizes must be used as intended and are non-transferable.

Exceptions
We understand that emergencies happen. If you need to cancel within 48 hours due to an emergency, please contact us as soon as possible to discuss your situation.
– We reserve the right to waive the cancellation fee on a case-by-case basis for extenuating circumstances.

Outstanding Payments and Collections
– Payment Obligations:
Payments owed to ICON Anti-Aging and Aesthetics PLLC are expected to be made promptly as agreed upon at the time of booking or membership enrollment.
– Collections Process:
If payment remains outstanding after three collection attempts, the balance will be referred to our collections agency partner. Clients will be responsible for any additional fees associated with the collections process.

By booking an appointment, enrolling in a membership, or participating in a promotion with ICON Anti-Aging and Aesthetics PLLC, you acknowledge and agree to the terms outlined in this Cancellation and Payment Policy.
